Order Form PDF I'm dedicated to providing my customers the best possible service and would love to hear from you. If you have any questions, please don't hesitate to call me or contact me via email 24 hours a day. I will assist you as soon as possible. Gift Services and Personalization For a $5 charge, I offer gift wrapping and personalized gift cards. When ordering, please specify in the "Message to Seller..." section on either the order form or PayPay checkout — specify exactly which items you need wrapped or cards for. I can also personalize any order, like monograms for baby, for a $25 charge. Just contact me with your request, making sure that you include your phone number, email, and any questions you may have. Custom Orders Each piece is unique, but I will discuss special orders on patterns, however, most vintage piece are one-of-a kind. A minimum order of fifty dollars will be required for special orders on oil cloth and pillows. Special order quilts will require a non-refundable deposit of 1/4 of the estimated cost to initiate project with the final 3/4 payment due prior to shipment. Lead-time on baby blankets is 8 to 12 weeks and 3-6 months for full-sized quilts. Please contact me with your request. Visiting the Studio A lot of the merchandise can be seen at my studio in Dallas. Please call me at 214.244.8824 or email me at judy@judyvetter.com where we can arrange an appointment. Wholesale Orders For wholesale inquiries, please send an email to judy@judyvetter.com. Order Policies Receiving Payment Judy Vetter accepts PayPal payments, money orders and checks (merchandise will be shipped only after check is cleared.) On money order and check orders, fill out the order form and send to 501 Second Ave., Bldg E, Dallas, Texas, 75226 or email me at judy@judyvetter.com. I will need to know the name, color and size of your selection(s) and I reserve the right to cancel orders that don't meet requirements. I encourage international customers to pay through PayPal. Sales Tax If you're a Texas state resident, you'll have to pay 8.25% in sales tax. The rest of you are responsible for complying with your state's current laws regarding internet purchases. About PayPal PayPal is a safe, secure, low-cost way to make credit card payments online. Please note that you don't need to create a PayPal account to use the PayPal shopping cart service. Simply click on the box in the middle of the PayPal prompt page once you've checked out — it says, "if you do not currently have a PayPal account click here." PayPal will take your credit card information for me. Shipping I ship UPS Ground or USPS and charge a flat fee for shipping anywhere in the U.S., Alaska, Hawaii, and U.S. territories. My shipping rates are based on total purchase amounts. Standard ground orders should arrive within 10 business days, which includes time to process your order. Please note that UPS does not deliver to P.O. Boxes. All orders will be shipped ground unless otherwise requested in the "Message..." field on the order form or during PayPal checkout.

Out-of-Stock Items If your selected merchandise is out-of-stock, I will inform you immediately upon receipt of order. You will be notified via email of shipping schedule. Merchandise Delays All items are displayed in good faith. Certain items, however, are subject to unexpected production delays. I will do everything possible to secure these items and fill your order as quickly as possible. Refunds and Returns I take pride in all Judy products and stand behind the quality of my merchandise. I try my best to describe the merchandise as accurately as possible and strive to make each a special piece of useable art. Due to the handmade and one-of-a-kind nature of the products, there will be slight differences in color, etc. If for any reason you are unsatisfied, please notify me within 7 days. I will either refund your or give you a credit (except on custom orders where the refund will be less the original non-refundable deposit). After the 7 days of required return notification, you will receive a credit only. Shipping costs are not refundable. Refunds will be made within 30 days of notification. If damage occurs during shipping, contact me as soon as possible and I will work with you to have the merchandise returned or replaced. Please keep all original packaging materials. Making a Return
